Cuba requires valid medical insurance from every traveller and checks it on entry, and you also need the online D'Viajeros form within 48 hours of landing. Note too that a visit to Cuba can disqualify you from a later US ESTA.

At some destinations, Nepal among them, the authority collects its fee itself at the airport counter. There you pay us the service fee only, and the fee stays with you for arrival.
